Boca defender Clemente Rodriguez was sent off on the hour after a second yellow card for verbally abusing referee German Delfino.
The 0-0 scoreline after normal time replicated the result at Boca's Bombonera stadium a week earlier. Under Conmebol rules, the match was decided on penalties with no extra time.
In Wednesday's other match, Olimpia of Paraguay defeated Brazil's Fluminense 2-1 and on aggregate to advance to the last four.
