Stefan Radu headed in for Lazio following a free-kick from Danilo Cataldi on the right flank four minutes after kick-off.
Giorgio Chiellini equalised in the 11th minute after Patrice Evra redirected a free-kick from Andrea Pirlo towards him at the edge of the box.
The 'Bianconeri' will achieve the treble of titles if they beat Barcelona in the Champions League final in Berlin on June 6.
This was Juventus' 10th Italian Cup title, also the first since 1995.
