The 45-year-old singer changed her surname after tying the knot with Marc Anthony, whose real name is Marco Muniz, in 2004.
However, court documents show that Jennifer has reverted to her original name on December 31, 2014, reported TMZ online.
Jennifer and Marc split in July 2011, with the 'On The Floor' hitmaker later dating dancer Casper Smart, who she separated from in the summer of 2014.
Jennifer recently admitted she felt like she was going to die when her marriage to Marc broke up.
