The elections will be held on November 22, 28, December 5 and 12, the State Election Commission said.
The model code of conduct has come into immediate effect, State Election Commissioner Shiv Basant told reporters here.
The counting for the first and second phases will be held on December 6 while the exercise for the last two phases will be carried out on December 13 and 19.
