DGCA observed lack of system to facilitate management of the distribution and revisions of operational documents, lack of man power and training, Minister of State for Civil Aviation K C Venugopal told the Upper House in a written reply to a query.
It also found that accident or incident reporting system of the airlines was not proper and lack of calibration of various maintenance and operational tools. Lack of adequate organisation procedures were not in place to ensure uniform standard of work, he said.
"All the deficiencies in the system found during the audit were taken up with the airlines," the minister said.
