The woman, Swathi (27), was married to Sudhakar Reddy (33), who was into stone-crushing business, and has two children by him.
She was in a relationship with the physiotherapist, identified as A Rajesh (30), according to police.
They allegedly killed Reddy at his house on November 27. The duo then took the body in Reddy's car to a forest in Mahabubnagar district, about 70 km from his house in Nagarkurnool, and set it on fire, police said.
Swathi then got Rajesh admitted in a hospital in Hyderabad claiming he was her husband, police said.
Rajesh was arrested from the hospital where he was undergoing treatment, Nagarkurnool circle inspector Srinivas Rao said.
According to police, Rajesh allegedly drugged Reddy with an injection on November 27 and then, along with the woman, hit him on his head with a sharp-edged object, killing him on the spot.
As a part of their conspiracy, Rajesh, to whom the woman got attracted when she visited his clinic two years ago, tried to "pass off" as the man who was killed, inspector Rao said.
The woman had plans to get a plastic surgery done on Rajesh, he said.
Rajesh and Swathi were allegedly having an affair since some time and wanted to live together by getting rid of her husband, he said.
However, Reddy's brother and other family members grew suspicious after seeing the behaviour and appearance of Rajesh, whose burns had healed by the time, at the hospital.
They approached the police last week complaining that the person was not Reddy. Following this police questioned Swathi, who allegedly confessed to the plot she hatched with Rajesh, Rao said.
Swathi was arrested on December 10.
The woman and Rajesh have been booked on charges of murder under relevant IPC sections, the police inspector added.
