There is a shortage of application forms for the Prime Minister's Crop Insurance Scheme in the Yavatmal district, an official said.
The last date for submission of form is July 31.
The district needs 4.5 lakh application forms whereas the total supply so far is around 1.25 lakh.

District Agriculture Superintending Officer Dattatraya Gaikwad confirmed the shortage.
Collector Sachindra Pratap Singh said he had taken up the matter with the higher authorities and sought extension of the period for submission.
Activist and Shri Vasantrao Naik Shethi Swalamban Mission president Kishor Tiwari has also demanded extension of the deadline.
