Fire truck lacked system to trigger runway alert in LaGuardia crash: NTSB

A runway warning system failed to sound an alarm before an Air Canada jet collided with a fire truck that crossed its path while landing at New York's LaGuardia Airport, federal investigators said Tuesday. National Transportation Safety Board (NTSB) investigators are looking at whether that lack of warning or any other problems with the airport's layers of safety precautions could have prevented the crash. Among the areas being explored are control tower staffing levels, who was in charge of coordinating the air and ground traffic and whether the fire truck heard the control tower's frantic, last-second warnings to stop. "We rarely, if ever, investigate a major accident where it was one failure," said National Transportation Safety Board chairwoman Jennifer Homendy. "When something goes wrong, that means many, many things went wrong." The Air Canada plane carrying more than 70 people slammed into the fire truck late Sunday, killing both pilots and injuring several passengers. Most,

2 pilots killed after jet collides with fire truck at New York Airport

An Air Canada jet carrying more than 70 passengers collided with a fire truck while landing at New York's LaGuardia Airport in the US late Sunday, killing the pilot and copilot and injuring several others, officials said. Transportation Secretary Sean Duffy said LaGuardia is "well-staffed" but still faces a shortage of air traffic controllers. He said there are currently 33 certified controllers but the goal is to have 37. More than one controller was on duty at the time of the accident, he said. "I can't give specifics on what went wrong," Duffy said, deferring to the National Transportation Safety Board, which is leading the investigation. Canada also sent a team of investigators. The impact of the collision severed the cockpit, and hurled a flight attendant - still secured to her seat - far from the site of the crash, her daughter told a Canadian TV station. The flight attendant survived. The fire truck was crossing the tarmac just before midnight after being given permission to

Jet aborts takeoff to avoid runway collision at New York's LaGuardia

Federal officials are investigating why two planes got dangerously close on a runway at New York's LaGuardia Airport earlier this month despite the airport being equipped with an advanced surface radar system that's designed to help prevent such close calls. Both the Federal Aviation Administration and the National Transportation Safety Board said Monday that they are investigating the May 6 incident when a Republic Airways jet had to abort takeoff and slam to a stop because a United Airlines plane was still taxiing across the runway. In audio from the tower that ABC obtained from the website www.LiveATC.net, the air traffic control said to the pilot of the Republic Airways jet: Sorry, I thought United had cleared well before that. At the time that controller was directing the Republic Airways jet to takeoff, a ground controller on a different radio frequency was directing the United plane to a new taxiway after it missed the first one it was supposed to use to exit the runway. Didn't intend to outrage complainant's modesty, accused man in Delhi court

Shankar Mishra, the man accused of urinating on an elderly woman co-passenger on an Air India flight, told a Delhi court on Wednesday his act was not driven by sexual desire nor aimed at outraging the complainant's modesty, as he sought bail in the case. The complainant's counsel opposed Mishra's bail application, claiming she was being threatened. I'm regularly receiving messages, threatening me. Accused's father sent me a message and said 'karma will hit you' and then deleted the message. They're sending me messages and deleting it. This needs to stop... Air India instead of separating the accused and complainant, tried to mediate the crime, the counsel said. Metropolitan Magistrate Komal Garg reserved the order on the bail application of Mishra. Nobody should have to go through something like that. It was revolting. I feel bad for the poor lady. It was appalling However, was the unzipping aimed at sexual desire? No. Was any of this act aimed at outraging her modesty? No, the ...

A Delhi court on Wednesday reserved its order on the bail application of Shankar Mishra, accused of urinating on an elderly woman co-passenger on an Air India flight. Metropolitan Magistrate Komal Garg reserved the order on the plea, which said the act was not driven by sexual desire and that it was not aimed at outraging the complainant's modesty. Another magisterial court had sent Mishra to 14-day judicial remand on Saturday, denying police his custody. The incident took place in the business class of the Air India flight from New York to Delhi on November 26 last year.
